How do I book an appointment at La Vida TeleCare?

To book an appointment, simply complete the necessary form on our website. After submitting the form, you will receive an email with details, including an intake form and access to our patient portal. This portal allows you to enroll, view your prescriptions, and manage your appointments—all in one place. Please prepare your documents and forms before your appointment time.

Do you accept insurance?

Yes, we accept various insurance plans. Please check with our office or your insurance provider for specific details.

Can I pay with cash?

We do not accept cash payments; however, we do accept online payments for your convenience.

What are the visit fees?

There is a $25 appointment booking fee. The remaining costs will be discussed during your treatment plan consultation.

Do I need any special equipment for my appointment?

No special equipment is required! All you need is a reliable internet connection and a smartphone, laptop, or you can even connect via phone call or text. You can choose the method that is most comfortable for you.

What is the appointment cancellation policy and what are the charges?

To avoid any cancellation fees, please ensure that you cancel your appointment at least 24 hours in advance. If you cancel your appointment within 24 hours of the scheduled time, a $50 cancellation fee will apply. Thank you for your understanding!
